After beating him you get the final ending, ending 16. In the door youll find Mega Satan, the true final boss. With the key pieces, go to the Chest/Dark Room to unlock the golden door. You need to blow up 2 Angel Statues and beat them to get two key pieces. In the Angel room, use a bomb to blow up the Angel Statue. In order to unlock that door, you need to skip Deals with the Devil in order to get Deals with the Angel.
Also, once you get to those places, youll see a big, golden door in the starting rooms. There, youll beat the respective end bosses and get an ending.
After beating Sheol and the Cathedral 6 times (i think) each youll unlock the final floors permanetly, the Chest and the Dark Room. Those floors are Sheol and the Cathedral. each time you beat Moms Heart you'll unlock a new item until you eventually unlock the upgraded version of the Heart, It Lives, and you unlock 2 new floors permanetly. Since you beat mom for the first time, you unlocked a new floor, the Womb. After You beat your first run you need to do another as any character.
